Here are some pics of the front mount and my issue.
This is a pic from the side. You can't really see the grill bowing out. The rusty bottom piece is what was attached to the back side of the grill.

You can see in this pic the "notches" where the cage nut piece WAS welded to.

Here's the piece after I grounded the failed welds.

My question... where does this piece get welded to? Same location?

I would suggest trying to make the connection as ridgid as you can. The lift still looks a bit on the tall side, with a tall body lift there can be issues where the steering rod connects at the upper steering shaft. When I installed my lift, this location required a bit of adjusting, since it would bind up when steering. At this point you should notice that the radiator will have to be lowered along with everything else that goes between the body and frame. Where the puck connects to the body is a good location for rust to form also.

I see what you're talking about with the radiator. I'll see how it cools first. The steering shaft was touching the exhaust manifold, but it clears now. I'll see how it does once I get it all back on the ground.
So in that first pic, that is how it's suppose to be? I'll just get everything to line up then weld that piece back to the grill? Sound like a plan?
 
I just did the same lift and my front nut cage under the radiator was disintegrated. I got everything else lined up and just welded that nut down without the cage. Here is a link with some good info and about all the adjustments that has been done with this lift.
