Looks like you got it; except for the right rear.

Still looks crooked like the leaf isn't in line with the frame or something?

Or it could just be that crooked bolt hole you are mentioning; maybe the bushings being all worn caused the hole to elongate. With that one being at an angle like that, its going to wear out really quick again.

Looks like you are on the right track though. Just gotta figure out what is causing that one to sit cockeyed.

Edit; blew up the picture and on the right rear it looks like the leaf isn't perpendicular to the axle. Like its not sitting right on the plate? Are there two U bolts on there; I can only see one. It may be the angle of the camera making it hard to see. Let us know.
 
yes 2 ubolts...what i notice is the hole on the plate is oval/worn?..so may be the spring maybe sitting over a bit...also me and another tech always talked about the tubes were welded a bit at the pumpkin and maybe bent or something....i have a set of axles...measured today at work seem the same....maybe off a tad...so gonna swap housing..so wanna do both..need time w/ this being my Daily...but gonna try and get shackles in the mean time..see if it helps...need to find good ones:D

someone probably welded them so they wouldn't spin a tube. It's a commom thing to do to a 20.
 
If the holes in the plate are too big/worn I could see that allowing the axle to move in relation to the leaf spring.
