I was looking last month and all I found was one from Delorean (yes that Delorean).
Has all 4 vac line hookups. Pricey at 100 bucks but so far so good. Their PN 101109.

The Delorean part is a popular replacement for Jeeps. It has the same 4 connections, just reversed (left to right).
