Sorry not much help but try reverse engineering with Novak?
 
IMO The Dana 300 would be your best bet.
 
Let's see if we can sort this out. Hopefully tour th350 is a 4x4 version either .750 or 5.50 measured from face of the Transmission 4 bolt flange to the end of the shaft lets hope for .750. Option 1 would be Transmission at 21.0 in factory gm adapter 3.65 in and np 205 at 14.0 in tot length 39.15 in cost around 250.00. Option 2 would be a Dana 300 but it's a bit salty at around 600.00 for the adaptation parts plus a Transfer Case overall length I think is a bit shorter around 36.4 . If your output shaft is different and or has a tail shaft housing it's a different ball o wax . Let us know and we can move from there. I'm going to go get another bowl of ice cream.
 
that is a long drivetrain, I would use a Dana 20 with that to keep things short
 
Yeah me too! I opted out of a 205 and went with a 300. And I've got a 7 not a 5. I've seen 350/205 combos with the 5.5 shaft in a 7 and the driveshaft angle was not pretty.
