Clutch Clutch and Brake Assembly Identification

Hello, I need some help identifing the attached clutch and brake pedal assembly. This is what came with my 1975 CJ5 , I did not remove it from the old metal tub but it looks like it was bolted in the firewall. I have attached a piece of the old firewall that looks like it was attached ot it. I will be using this firewall piece as a templet on my newer glass tub. The assembly works ok from what I can see but, there is a piece missing that attaches to the dash. I have also attached a picture of the upper assembly support I just bought for this assembly so I can bolt it in place. As you can see the support is not correct. Any help would be great, thanks.
