I did both in different spots.
For the floor where your feet go (which always seems to be gone) I just cut it out well past the rust. Where it curved up in the front it overlaps just a bit. Where it meets the side of jeep it meets flush, same towards the back where it meets the tool box under the seat.
Mine was all rusted out over the rear wheel wells as well. I cut/knocked all the rust out. Put down some rush inhibitor. Then welded a piece of metal over that I made from a cardboard template.
As far as brackets go; if you look under the jeep you'll see some C channel that the floor is essentially sitting on. A bunch of mine was all rotted out so it had to go. I used a mix of C channel and square tubing to replace that as well as make new mounts for the body mounts.
I'm not good enough to make it all pretty so I went with the armored tank look.
