Is that an aluminum axle degree shim? If so, over time they compress and your U-bolts loosen.

Steel diff shims do not wear out.
 
I don’t think it’s aluminum, definitely time for an upgrade on the u bolts I have the big 5/8 bolts in the rear but 1/2 u bolts on the front, I was pretty damn sure they were tight, I find it odd that I have a shackle flip and it still bent
